Why should I go to St. Camillus?

St. Camillus is in the business of putting people back in control of their lives. Each patient receives an individualized schedule of structured programs seven days a week. This allows individuals to quickly return to their highest level of physical and emotional functioning.

Who will I be treated by?

Treatments and services are provided by the RehabPath team, a dedicated staff of highly-skilled rehabilitation professionals. The same team members will work with you and treat you throughout your entire RehabPath stay. These team members have worked with many individuals like you with similar rehabilitation goals to achieve the best possible outcomes.

What can I expect on the day of admission?

Upon arrival, insurance information will be obtained and admission documents signed. (This may be done one or two days prior to admission. For more information, please contact St. Camillus' inpatient admissions personnel or your doctor.) You and your family will then be escorted to the RehabPath Program unit, and staff introductions will be made with the RehabPath team.

Will my insurance cover my stay?

St. Camillus will contact your insurer to determine your coverage upon referral to the RehabPath.

During your stay your case manager will remain in contact with your insurer. You will be informed of these contacts and advised of any changes regarding your coverage.

What services are available?

In addition to the therapy, medical and nursing care that is necessary for your rehabilitation, you may also utilize St. Camillus' other support services as needed during your stay. They include:

St. Camillus is a unique healthcare facility in that it operates a continuum -- or integrated system -- of care. To further rehabilitation progress after discharge, you may access a wide range of on-site support programs and services as needed . They include most of the above-mentioned services, plus the following:

Outpatient nursing services | Outpatient rehabilitation -- PT, OT, speech therapy
Social services | Vocational rehabilitation counseling

 

How can I get to an outside medical appointment?

Transportation to outside physician/clinic appointments can be arranged by your case coordinator/social worker. If desired, St. Camillus' on-site Transportation Services department offers a fleet of vans staffed by qualified drivers for transportation to your appointments.

When can I have visitors?

Visiting hours are 8 a.m. to 8 p.m. daily with no age restriction. Pet visitation is permitted as long as the pet is on a leash. Visits from family and friends are strongly encouraged.

When can I leave the facility on a pass?

Therapeutic passes require the physician's and your insurance company's approval. Family training is provided before the therapeutic pass is issued to ensure a safe and pleasant outing.

Who will help me plan for my discharge?

Length of stay is individually determined and based on your functional goals as developed by the team with your cooperation.

Discharge planning is a patient/family/team process that begins on your day of admission. The team will make recommendations for discharge needs such as equipment, assistive devices and outpatient rehabilitation services. Your case coordinator/social worker will work closely with you and your family to plan your discharge and arrange for any needed follow-up services and facilities.

Preparing For Your Stay

Once you have been notified of your acceptance to St. Camillus, it is time to prepare for your stay.

A bag should be packed with the following items:

  • Toiletries (shavers, toothbrush, toothpaste, etc.)
  • Several sets of night clothes (sleeping attire, slippers)
  • Several sets of daytime clothes (comfortable items that do not zip or snap, i.e. sweat suits)
  • Comfortable shoes with low heels (i.e. Velcro sneakers)
  • Phone, television, VCR, radio, etc.
  • Personal items which are comforting such as bed spreads, desktop photographs, etc. (label everything, especially clothing if you prefer to use the facility laundry)

Personal expense accounts

You may open a personal account (for in-house expenses such as gift shop purchases, beauty shop/barber services, and for safekeeping cash) with the receptionist located in Building II. Account deposits may be made with the receptionist anytime she is on duty, seven days a week. Account withdrawals may be made with the receptionist weekdays from 10:00 a.m. to 12:00 p.m. and 1:00 p.m to 3:00 p.m.

St. Camillus RehabPath Team

Mission:
To provide high-quality, cost-effective, restorative services that promote achievement of the highest level of physical, mental and psychosocial abilities possible for individuals admitted to our care.

Case manager
The case coordinator and social worker are available as a resource for referrals, education and advocacy. They assist patients and family throughout the admission, treatment and discharge processes; function as a liaison between the patient, family and insurer; and provide counseling to patients and families as needed.

Dietitian
This team member evaluates and monitors the dietary status of the patient. Working closely with the physiatrist and nursing staff, the dietitian creates an individualized program to meet the patient's nutritional needs, including tube feedings and specialized diets if necessary.

Family
Family members and significant others are very important parts of the rehabilitation team. They are a resource for support and encouragement. Because the patient receives emotional support when the family is actively involved in the treatment process, we involve the family in all aspects of the recovery.

Occupational therapist
Occupational therapists focus on improving independent living skills such as bathing/toileting, dressing, grooming, meal preparation and money management while jointly addressing physical and cognitive factors limiting patient self-sufficiency. They also addresses problems with upper extremity (arm/hand) function and incorporate assistive aids as needed to help with independence.

Physiatrist (attending physician)
The physiatrist-a physician specializing in physical medicine and rehabilitation-oversees the patient's rehabilitation program and directs their complete medical care so they maintain medical stability and good general health. The physiatrist works with the therapy team to develop and implement an optimal package of rehabilitative services.

Physical therapist
Helping the patient maximize physical independence using a regimen of physical therapy treatments and exercises is the work of the physical therapist. Balance, coordination, flexibility, strength and endurance are addressed to improve the patient's functional mobility whether in bed, a wheelchair or walking.

Rehabilitation nurse
The rehabilitation nurse provides skilled nursing care and education, promoting patient wellness and safety. The nursing staff works with other team members, encouraging patients to use and practice new skills learned in therapies. This helps return the patient to their prior lifestyle quickly.

Speech-language pathologist
This professional identifies and develops the best means of communication for patients as needed. Treatments address impairments in speech and language as well as related underlying thinking skills. The speech-language pathologist is also able to evaluate and treat swallowing disorders.

Therapeutic recreation specialist
Therapeutic recreation specialists provide regularly-scheduled therapy sessions, recreation resources and opportunities to maximize patient functioning. They also provide education and community-reintegration activities.

 

Getting started

Entering the RehabPath Program and charting a course to better health is easy.

First, the individual and/or the family should speak with the doctor, hospital discharge planner, or St. Camillus' inpatient admissions personnel. They will help determine whether the RehabPath is suitable for both the patient and family. This can usually be done either before or immediately after the patient undergoes surgery or other initial treatments.

After deciding to refer an individual to the RehabPath Program, the doctor or discharge planner will make the referral. Then the caring, professional staff at St. Camillus will take care of everything else -- including any insurance coverage issues. It's that simple!
