Services provided
Medical management: RNs/CNAs monitor and assess health status and provide skilled nursing services as needed.
Case management: Promotes coordination and communication within the program and between community services to ensure quality/efficiency of services.
Therapies: Restorative therapies such as physical, occupational and speech/language therapy, and vocational rehabilitation/counseling.
Education/support: Information provided to registrants and caregivers on health issues, community resources, emotional and psychological support and respite. We also host a Multiple Sclerosis Support Group that meets monthly.

Hours
- 8:00 a.m. to 4:00 p.m. Monday through Friday
Registrants may attend a minimum of three hours one day per week, up to eight hours five days per week.
Cost
- $103 per day not including transportation
Medicaid covers the cost of the Day Health Program and transportation. Medicare and private insurance generally do not cover the cost of the program or transportation. Private pay is accepted.
If program costs are a concern, please contact Lisa Santmyer (see referrals below) to see if an alternative payment schedule can be arranged.
Admissions/Referrals
Generally, a home evaluation is conducted. If appropriate, the registrant can begin within 24 hours of the home visit.
