Nursing Services

LPN
Seeking LPN with at least six months experience. Under the direction of the head nurse or charge nurse, assists with the preparation of nursing care plan and acts as a team leader in the implementation of the plan to provide high quality care to residents within assigned unit. Conform with State and Federal Health and Safety regulations and practices. FT/PT 3:00 PM - 11:00 PM, 11:00 PM - 7:00 AM. Friday/Saturday/Sunday OR Saturday/Sunday/Monday 7:00 AM - 7:00 PM and 7:00 PM - 7:00 AM.

RN Supervisor
An opportunity to expand your nursing career by using your extensive nursing education and experiences. Develop your leadership and management experience in a long-term care facility. Accountable for the supervision of patient care within LTC/brain injury/subacute units. Current NYS RN License and two years of experience. CPR/AED certified. Full-time or Part-time, 3:00 – 11:00 P.M. with some 11:00 – 7:00 A.M. Monday - Friday, no weekends!

Communications Operator
This position operates the switchboard, paging system, and provides information, directions, and/or assistance to the general public, residents and staff; acts as a receptionist and performs a variety of general business office and accounting functions as assigned by the President and Comptroller. Maintains a pleasant and courteous attitude. Must possess a high school diploma or its equivalent. Strong verbal and interpersonal skills required. Previous switchboard experience is also required. Part-time. Every Wednesday 4 – 10 p.m. and every Sunday 2 – 10 p.m. and as needed other days. Requires occasional holiday.

Unit Nurse Manager for the Brain Injury Program
FT position responsible for planning, organizing, developing and managing the day – to – day operations in accordance with current regulatory requirements. The position will further develop the cross functional and accountable team by encouraging communication, dedication to customer service and a commitment to quality improvement. In addition, maintain oversight for the provision of the coordination, implementation and review of clinical care along with the effective management of the clinical documentation standards, employee engagement, clinical performance outcomes, safety culture, unit metrics and etc. The leader will maintain a patient focused caring atmosphere while satisfying the patients’ needs in conjunction with fostering an environment of collaboration and respect. Must possess a current Licensed as a Registered Professional Nurse from New York State. A strong nursing background in either Medical Surgical, Neurology or Traumatic Brain Injury Program is required.

Director of Clinical Services (Reports to the Director of Nursing)
Provides strong leadership and delivers strong clinical capabilities, organizational and communication skills. The core responsibilities include the following: 1) provide oversight to two continued care units; 2) develop and maintain nursing policies and procedures; 3) manage the facility’s Infection Control and Skin / Wound Care Programs; 4) function as the department’s liaison with the radiology, laboratory and pharmacy vendors; 5) manage the Incident Reporting Program; 6) manage the department’s quality assurance program. In addition, the position
contributes to ensure the service culture is aligned to foster the best customer experience, achieve desirable DOH outcomes, sustain a culture that fosters employee engagement, achieve the desired clinical outcomes and expand our clinical services to align with the future health care needs. Current NYS RN license. BSN preferred. Three years clinical and comparable leadership experience preferred.

Nurse Aide Training Program Coordinator/Instructor
This position supervises all aspects of the NATP and reports to the Director of Staff Education. This position is responsible for the day-to-day operations of the training program in accordance with policies and procedures and State and Federal requirements. Strong verbal, written, computer and leadership skills required. FT, Monday-Friday. Current NYS RN License and two years related experience working in a nursing home and able to demonstrate competency to teach adult learners as evidenced and documented by at least one of the following: Completion of a professionally recognized course in teaching adult learners or NYS Education Department teacher certification; two years of experience teaching nursing or nursing related programs to adults in an academic setting approved by the State Education Department or other recognized accrediting body; two years of experience teaching nurse aides in a residential health care facility.

Insurance and Billing Coordinator
Responsible for coordinating day to day insurance and billing functions necessary to assure compliance with federal and state regulations. Must possess, at a minimum, a two-year college degree or significant experience in health care insurance and billing functions. Strong verbal, written, computer and leadership skills required. Full-time, M-F

Unit Helper
Provides support on the unit such as but not limited to patient transportation, distribution of supplies, support to the provision of care, etc. Will also assist unit staff with duties that do not require specific clinical licensure or certification. Must have a strong work ethic and be able to work without constant supervision. Employees should be able to read and follow directions of direct care staff. 7:30 a.m. – 3:30 p.m. every other weekend (part-time) OR 3:30 – 7:30 p.m. one week night and every other weekend (part-time).

Day Health

Adult Day Health Program RN
Manages/supervises a team of full time CNAs and LPN in the ADH Program. Must possess strong leadership, communication, organizational, assessment, care planning and computer skills. Current NYS RN License and two years related experience working in a healthcare facility. Strong verbal, written, computer and leadership skills required. UAS experience preferred. Part-time position.

Medical Day Program Activities Leader
Coordinates the social, intellectual, cultural, educational and physical group activities. Plans, organizes, and leads group activities and individual interventions designed to meet the needs and interests of the Program participants. Prefer, but not necessary an Associate’s Degree in Recreation or related field. Verbal, written, and computer skills required. Ability to demonstrate enthusiasm for activities. FT M-F 8- 4:30 p.m.

Dining Services

Cook
FT position. Prepares foods according to recipes and production tallies and in accordance with all food safety and sanitation regulations. This position works closely with the Executive Chef, as part of a team to provide nutritionally adequate meals to Residents and Patients of the facility. There is a minimum of a high school diploma or equivalent. This position requires the ability to read and write English and to have basic math skills. A minimum of two years experience required.

Staff Home Health Aide
Covers HHA level home care patients on a daily basis for permanent short term cases and to cover vacations and staff call-offs, assists in the office as deemed necessary by the Director of Patient Services. Guaranteed 30 plus hours per week, flexible schedule, on call two weekends per month. Must have reliable transportation and good communication skills. Current NYS CHHA certification.

Home Care Screener
Screens patient referrals from acute care facilities, skilled nursing facilities and physician offices for admission to St. Camillus Home Care and Inpatient programs. Also completes a thorough review and timely transfer of medical record information to expedite the admission decision-making process. A minimum of two year professional nursing experience in an acute care or rehab setting, home care experience preferred; knowledge of third party payer regulations, including Medicare and Medicaid; excellent assessment and strong verbal and written communication skills; basic computer skills; and must have a valid NYS Driver’s License and NYS RN License.

RN Case Manager
Provides and coordinates care to TBI Waiver patients in the home setting utilizing the nursing process of assessment, planning, implementation and evaluation of safe and therapeutic care. Must possess, at a minimum, graduation from an accredited School of Nursing. Current NYS RN licensure. Prefer 2 years of nursing experience. Prefer home care experience. Strong verbal and written communication skills. Basic computer skills preferred. Must possess valid NYS drivers license and current automobile insurance and able to drive during work hours. Full-time or Part-time.

Transitional Care Physical Therapist
FT position providing transitional therapeutic care for patients in need of inpatient, home care and outpatient physical therapy services. The therapist will evaluate, organize and conduct physical therapy programs in multiple care settings to maximize patient’s potential for independence in an expected manner. One year of clinical experience and New York State License in Physical Therapy is required. New York State Driver License.

Home Care On-Call Nurse
Supports patient care needs of the agency during non-business hours. Also provides telephone triage for patient or staff related issues. Current RN license, BSN preferred. Must possess, at a minimum, 4 years related experience working in a healthcare facility or home care agency. Must possess a strong level of independence, dependability and autonomy. Must show a clinical experience of medical surgical nursing care and case management. Supervisory experience recommended but not required. Full-time, 4:30 pm – 8:00 am (some weekdays, weekends and holidays required).

After Hours On-Call Coordinator
Supervisory position that will utilize telephone assessment skills to triage and determine patient needs, take appropriate action in the identified need i.e. scheduling visit of staff, contacting MD or sending the patient to the ED. The qualified candidate will provide appropriate follow up as necessary, facilitate care coordination as required and guide nursing personnel in providing high quality patient care in the home care setting during non-business hours. BSN in nursing with at minimum 2 years experience in home care or an associate’s degree with 5 years experience in home care. 4:30 p.m. – 8:00 a.m. Part- Time.

Home Care Speech-Language Pathologist
Assess and treat adults with neurogenic communication disorders, dysphagia and cognitive impairments. All assessments and treatments are completed in patient’s homes. Participate in program development, staff education and interdisciplinary process. Work closely as an interdisciplinary team member across all programs within St. Camillus. MA/MS in Speech / Language Pathology with CCC. Experience with adult neurogenic communication disorders, independent performance of clinical swallowing assessment, identification and treatment of cognitive disorders. Strong CFY candidates considered. FT position with flexible hours.

Patient Financial Services

RN Case Manager (Managed Care Specialist)
Provides comprehensive care coordination for patients across the health care continuum to achieve the optimal clinical, financial, operational and satisfaction outcomes. Oversees and coordinates case management services, evaluates inpatient admissions for appropriate level of care determination, actively participates in the concurrent review process by communicating and intervening with Managed Care Organizations to ensure reimbursement for services provided while maintaining the integrity of care, collaborates with physicians, nursing, therapy, social work and other members of the interdisciplinary team to operationalise a single plan of care. Provides leadership to the interdisciplinary team with regards to the Managed Care Organization’s clinical requirements for specific levels of care, including documentation education for all disciplines, and monitoring of the appropriateness of the plan of care as it relates to length of stay and the discharge plan. Current NYS RN License required – BSN preferred. 2 to 4 years professional nursing experience in an acute care or rehab setting with case management/discharge planning experience; Certified Case Manager (CCM) desirable; knowledge of Medicare Advantage, Managed Long Term Care, and third party payer regulations; excellent assessment and strong verbal and written communication skills; basic computer skills.

TBI Waiver Independent Living Skills Training & Development Specialist
Works with individuals with a traumatic brain injury to assess functional abilities and identify and develop strategies to increase independence in a variety of living skills. Functional skills areas for development include personal and instrumental activities of daily living. PT with the possibility for FT hours. A master’s or advanced degree in a human service field and 1 year experience OR, A bachelor’s degree and 2 years experience OR, An Associate’s degree and 3 years of experience OR, A high school diploma and 4 years of experience. Experience must be in completing functionally bases assessments, developing comprehensive treatment plans and teaching individuals with disabilities to be more functionally independent.

Occupational Therapist
FT position in our team oriented Rehab Program, serving inpatient sub-acute rehab patients. BS in Occupational Therapy. NYS Licensure or pending licensure. OTR/L with experience in rehabilitation required. Excellent interpersonal skills required. New graduates encouraged to apply!

Rehabilitation/Mental Health Counselor
Provides rehabilitation and mental health counseling services to in-patients and out-patients. Primarily works with adult patients with brain injuries in the brain injury unit, but may also work with other adult patients with mental health issues. Must have a Masters Degree in Rehabilitation Counseling or Mental Health Counseling. Must be a Certified Rehabilitation Counselor (CRC) or Licensed Mental Health Counselor (LMHC). Prefer minimum of 2 years experience as a Rehabilitation Counselor or LMHC. Must be a Certified Rehabilitation Counselor (CRC) or Licensed Mental Health Counselor (LMHC). Prefer a minimum of 2 years experience as a Rehabilitation Counselor or LMHC. Prior experience working with persons with brain injury preferred. PT, weekdays, 20 hrs/week.

Social Services

Director of Social Services
Responsible for the administration of the daily operations of the department and provides strong leadership in respect to staff development, team work, guidance and support, goals, safety culture, establishing desired clinical outcomes, compliance, etc. and monitoring key metrics. Promotes innovation in delivering services. Ensures all aspects of discharge planning are effectively managed and the operation is appropriately positioned for the transition to managed Medicaid. Develops and implements performance improvement plans as required. Effectively acts on the department’s strategic vision that is aligned for anticipated changes in the health care industry and manages departmental policies / procedures, budget, MDS and care planning components associated with the psychosocial management, customer satisfaction, documentation, etc. Bachelors degree in related field, MSW preferred. Minimum of 3 years management / leadership experience required. Also must have 1 years experience in LTC setting, previous experience with case management with emphasis on discharge planning. Good written and oral communication skills required. Must possess a strong understanding of regulatory requirements and the provision of quality care & services, and be able to achieve desired outcomes.
