Nursing Services

Seeking LPN with at least six months experience. Under the direction of the head nurse or charge nurse, assists with the preparation of nursing care plan and acts as a team leader in the implementation of the plan to provide high quality care to residents within assigned unit. Conform with State and Federal Health and Safety regulations and practices. FT/PT 3:00 PM - 11:00 PM, 11:00 PM - 7:00 AM. Now hiring for 12 hour shifts. Friday/Saturday/Sunday OR Saturday/Sunday/Monday 7:00 AM - 7:00 PM and 7:00 PM - 7:00 AM.

RN Supervisor
An opportunity to expand your nursing career by using your extensive nursing education and experiences. Develop your leadership and management experience in a long-term care facility. Accountable for the supervision of patient care within LTC/brain injury/subacute units. Current NYS RN License and two years of experience. CPR/AED certified. Part-time, 3:00 – 11:00 P.M. with some 11:00 – 7:00 A.M.

RN Charge Nurse
Under the general direction of the unit Nurse Manager, and in accordance with state and federal regulations, assists in facilitating, and implementing through direct oversight the delivery of quality nursing care to the patients/residents of St. Camillus. Current NYS RN with 3 years health care experience with demonstrated leadership skills and management skills. Knowledge of systems, and operations preferred along with knowledge of federal and state regulations. Strong communication skills a must. Part-time, weekends 8-4 pm.

Nursing Services Scheduling Coordinator
Coordinates the staffing for all nursing units to ensure adequate and accurate staffing at all times. Coordinates with designated staffing agencies to support this endeavor. High School diploma or GED and strong verbal, written, and strong computer skills required. Full-time, days. Includes every other weekend.

RN Assessment Coordinator
Manages the MDS process in accordance with current federal and state guidelines, ensures completion and accuracy of the RAI. Contributes to the care planning and the quality improvement process. Provides ongoing staff education related to the MDS coding to ensure compliance and accuracy with documentation. Provides RN Unit Manager support as required in regard to clinical assessments, unit leadership, and operational management. Current NYS RN license and 3 years of RN experience, at least 1 year experience with MDS that includes knowledge and application of the 3.0 process. A working knowledge of the MDS/PRI process required, including a track record of maximizing reimbursement and regulatory compliance. Basic computer skills.

HHA Aide
Integrity Home Care Services is looking for compassionate, reliable, experienced, patient, and caring individuals to care for patients in their homes. Individuals will work with our Home Care staff. Training is provided for HCSS certification to work with patients with brain injuries as well. If interested, we encourage you to apply today!

Staff Home Health Aide
Covers HHA level home care patients on a daily basis for permanent short term cases and to cover vacations and staff call-offs, assists in the office as deemed necessary by the Director of Patient Services. Guaranteed 30 plus hours per week, flexible schedule, on call two weekends per month. Must have reliable transportation and good communication skills. Current NYS CHHA certification.

Home Care Screener
Screens patient referrals from acute care facilities, skilled nursing facilities and physician offices for admission to St. Camillus Home Care and Inpatient programs. Also completes a thorough review and timely transfer of medical record information to expedite the admission decision-making process. A minimum of two year professional nursing experience in an acute care or rehab setting, home care experience preferred; knowledge of third party payer regulations, including Medicare and Medicaid; excellent assessment and strong verbal and written communication skills; basic computer skills; and must have a valid NYS Driver’s License and NYS RN License.

Home Care Nurse Manager
Manages/supervises a team of full time and per diem Registered Nurses and LPN's in homecare. Must possess strong leadership, communication, organizational and assessment skills, strong computer skills are also a plus. Current RN license, Bachelors degree Required. A minimum of 1 year of home care experience, prior supervisory experience and experience with clinical oversight, quality assurance and over all case management. FT, M-F.

Home Care Assistant Nurse Manager
Provides assistance to the RN Manager in providing leadership and management of the Nursing team to assure that the optimal achievable quality of care is provided to the patients by the nursing staff. Nurse management will be in accordance with federal, state, and local standards, guidelines, and regulations that govern the facility and as directed by the Director of Patient Service or designee. Must be a graduate of an accredited School of Nursing and possess current licensure as a Registered Professional Nurse. Must possess at a minimum, 2 years related experience working in home care. Baccalaureate degree in a health or human services field and/or associate’s degree in nursing acceptable. Strong verbal, written, computer and leadership skills required.

Rehab Services

TBI Waiver Independent Living Skills Training & Development Specialist
Works with individuals with a traumatic brain injury to assess functional abilities and identify and develop strategies to increase independence in a variety of living skills. Functional skills areas for development include personal and instrumental activities of daily living. A master’s or advanced degree in a human service field and one year experience, or a bachelor’s degree and two years experience, or an associate’s degree and three years of experience, or a high school diploma and four years of experience. (Experience must be in completing functionally bases assessments, developing comprehensive treatment plans and teaching individuals with disabilities to be more functionally independent.) Part-time position with the possibility for full-time.


Van Driver
Full-time position responsible for driving and loading and unloading of wheelchair and stretcher bound patients. Must have at least three years of driving experience, be 21 years of age, and possess a CDL license with P endorsement.
