Duties include assessment of patients in their homes, clinical supervision of nurses and HHAs, staff training, orientation, in-services, and quality assurance. Must have a current RN license and two years of experience. Part time or Per Diem position.
Staff Home Health Aide
Covers HHA level home care patients on a daily basis for permanent short term cases and to cover vacations and staff call-offs, assists in the office as deemed necessary by the Director of Patient Services. Guaranteed 30 plus hours per week, flexible schedule, on call two weekends per month. Must have reliable transportation and good communication skills. Current NYS CHHA certification.
TBI Waiver Independent Living Skills Training & Development Specialist
Works with individuals with a traumatic brain injury to assess functional abilities and identify and develop strategies to increase independence in a variety of living skills. Functional skills areas for development include personal and instrumental activities of daily living. A master’s or advanced degree in a human service field and one year experience, or a bachelor’s degree and two years experience, or an associate’s degree and three years of experience, or a high school diploma and four years of experience. (Experience must be in completing functionally bases assessments, developing comprehensive treatment plans and teaching individuals with disabilities to be more functionally independent.) Part-time position with the possibility for full-time.
FT position in our team oriented Rehab Program, serving inpatients for inpatient continuing care rehab. BS in Occupational Therapy. NYS Licensure or pending licensure. OTR/L with experience in rehabilitation required. Excellent interpersonal skills required.
TBI/NHTD Waiver Service Coordinator
Facilitates and supports the services needed and desired by the participant to attain and maintain independent living in the community. A master’s or advanced degree in a human service field and one year experience, OR a bachelor’s degree and two years experience, OR an Associate’s degree and three years of experience, OR a high school diploma and four years of experience. FT, Monday – Friday (The Department of Health requires that the Service Coordinator to be contacted whenever an emergency occurs with one of their waiver participants. This may occur during nights and weekends)
Full-time position responsible for driving and loading and unloading of wheelchair and stretcher bound patients. Must have at least three years of driving experience, be 21 years of age, and possess a CDL license with P endorsement.
