Nursing Services

Seeking LPN with at least six months experience. Under the direction of the head nurse or charge nurse, assists with the preparation of nursing care plan and acts as a team leader in the implementation of the plan to provide high quality care to residents within assigned unit. Conform with State and Federal Health and Safety regulations and practices. FT/PT 3:00 PM - 11:00 PM, 11:00 PM - 7:00 AM. Now hiring for 12 hour shifts. Friday/Saturday/Sunday OR Saturday/Sunday/Monday 7:00 AM - 7:00 PM and 7:00 PM - 7:00 AM.

RN Supervisor
An opportunity to expand your nursing career by using your extensive nursing education and experiences. Develop your leadership and management experience in a long-term care facility. Accountable for the supervision of patient care within LTC/brain injury/subacute units. Current NYS RN License and two years of experience. CPR/AED certified. Full-time or Part-time, 3:00 – 11:00 P.M. with some 11:00 – 7:00 A.M. Monday - Friday, no weekends!

Communications Operator
This position operates the switchboard, paging system, and provides information, directions, and/or assistance to the general public, residents and staff; acts as a receptionist and performs a variety of general business office and accounting functions as assigned by the President and Comptroller. Maintains a pleasant and courteous attitude. Must possess a high school diploma or its equivalent. Strong verbal and interpersonal skills required. Previous switchboard experience is also required. Part-time. Every Wednesday 4 – 10 p.m. and every Sunday 2 – 10 p.m. and as needed other days. Requires occasional holiday.

RN Assessment Coordinator
FT position responsible for the management of the MDS process in accordance with current federal and state guidelines . Ensure completion and accuracy of the RAI. Contributes to the care planning and the quality improvement process. Provides ongoing staff education related to the MDS coding to ensure compliance and accuracy with documentation. Provides RN Unit Manager support as required in regard to clinical assessments, unit leadership, and operational management. Current NYS RN license and 3 years of RN experience, at least 1 year experience with MDS that includes knowledge and application of the 3.0 process. A working knowledge of the MDS/PRI process required, including a track record of maximizing reimbursement and regulatory compliance. Basic computer skills.

Permanent Charge LPN
Assists in the coordination of all nursing activities for residents on the assigned unit, in accordance with federal, state, and local standards, guidelines, and regulations that governs the facility and as directed by the Unit Nurse Manager or Director of Nursing or designee. NYS Licensed Practical Nurse. 1 + years LPN experience. Full-time, 3 p.m. – 11 p.m.

Day Health

Adult Day Health Program RN
Manages/supervises a team of full time CNAs and LPN in the ADH Program. Must possess strong leadership, communication, organizational, assessment, care planning and computer skills. Current NYS RN License and two years related experience working in a healthcare facility. Strong verbal, written, computer and leadership skills required. UAS experience preferred. Full-time, Monday – Friday. 8 a.m. – 4:30 p.m.

Dining Services

Prepares foods according to recipes and production tallies and in accordance with all food safety and sanitation regulations. This position works closely with the Executive Chef, as part of a team to provide nutritionally adequate meals to Residents and Patients of the facility. There is a minimum of a high school diploma or equivalent. This position requires the ability to read and write English and to have basic math skills. A minimum of one year of cooking experience. Part-time, hours vary.

HHA Aide
Integrity Home Care Services is looking for compassionate, reliable, experienced, patient, and caring individuals to care for patients in their homes. Individuals will work with our Home Care staff. Training is provided for HCSS certification to work with patients with brain injuries as well. If interested, we encourage you to apply today!

Staff Home Health Aide
Covers HHA level home care patients on a daily basis for permanent short term cases and to cover vacations and staff call-offs, assists in the office as deemed necessary by the Director of Patient Services. Guaranteed 30 plus hours per week, flexible schedule, on call two weekends per month. Must have reliable transportation and good communication skills. Current NYS CHHA certification.

Home Care Screener
Screens patient referrals from acute care facilities, skilled nursing facilities and physician offices for admission to St. Camillus Home Care and Inpatient programs. Also completes a thorough review and timely transfer of medical record information to expedite the admission decision-making process. A minimum of two year professional nursing experience in an acute care or rehab setting, home care experience preferred; knowledge of third party payer regulations, including Medicare and Medicaid; excellent assessment and strong verbal and written communication skills; basic computer skills; and must have a valid NYS Driver’s License and NYS RN License.

Home Care Nurse Manager
Manages/supervises a team of full time and per diem Registered Nurses and LPN's in homecare. Must possess strong leadership, communication, organizational and assessment skills, strong computer skills are also a plus. Current RN license, Bachelors degree Required. A minimum of 1 year of home care experience, prior supervisory experience and experience with clinical oversight, quality assurance and over all case management. FT, Monday – Friday.

RN Case Manager
Provides and coordinates care to TBI Waiver patients in the home setting utilizing the nursing process of assessment, planning, implementation and evaluation of safe and therapeutic care. Must possess, at a minimum, graduation from an accredited School of Nursing. Current NYS RN licensure. Prefer 2 years of nursing experience. Prefer home care experience. Strong verbal and written communication skills. Basic computer skills preferred. Must possess valid NYS drivers license and current automobile insurance and able to drive during work hours. Full-time or Part-time.

Transitional Care Physical Therapist
FT position providing transitional therapeutic care for patients in need of inpatient, home care and outpatient physical therapy services. The therapist will evaluate, organize and conduct physical therapy programs in multiple care settings to maximize patient’s potential for independence in an expected manner. One year of clinical experience and New York State License in Physical Therapy is required. New York State Driver License.

Rehab Services

Physical Therapist
Provides services to patients with orthopedic and/or medical problems who have functional losses. Doctoral degree (preferred) or Masters in Physical Therapy. NYS licensure or eligibility. Temporary position expected to last from November – April. Full-time or Part-time days, occasional weekend coverage.

TBI Waiver Independent Living Skills Training & Development Specialist
Works with individuals with a traumatic brain injury to assess functional abilities and identify and develop strategies to increase independence in a variety of living skills. Functional skills areas for development include personal and instrumental activities of daily living. PT with the possibility for FT hours. A master’s or advanced degree in a human service field and 1 year experience OR, A bachelor’s degree and 2 years experience OR, An Associate’s degree and 3 years of experience OR, A high school diploma and 4 years of experience. Experience must be in completing functionally bases assessments, developing comprehensive treatment plans and teaching individuals with disabilities to be more functionally independent.

Physical Therapist
Provides services to patients with orthopedic, cognitive, and/or medical problems who have functional losses. Doctoral degree (preferred) or Masters in Physical Therapy. NYS licensure or eligibility.
