Seeking LPN with 1+ years experience. Under the direction of the head nurse or charge nurse, assists with the preparation of nursing care plan and acts as a team leader in the implementation of the plan to provide high quality care to residents within assigned unit. Conform with State and Federal Health and Safety regulations and practices.

CNA Career Opportunity

Integrity RN Case Manager/Per Visit Nurse
Part-time or per diem position in our growing home care agency. Responsible for supervising, instruction, and guiding nursing personnel in providing high quality nursing services, for individuals and families in their place of residence. Current NYS RN license required.

Occupational Therapist
Responsible for providing the highest degree of quality care and services. Bachelor’s Degree in OT from an accredited college/university required. Must have training in rehabilitative and restorative therapy. Recent graduates are welcome and encouraged to apply!

TBI Waiver Independent Living Skills Training & Development Specialist
Per Diem position working with individuals with a traumatic brain injury to assess functional abilities and identify and develop strategies to increase independence in a variety of living skills. Functional skills areas for development include personal and instrumental activities of daily living. A master’s or advanced degree in a human service field and 1 year experience, or a bachelor’s degree and 2 years experience, or an Associate’s degree and 3 years of experience, or a high school diploma and 4 years of experience.

Staff Home Health Aide
Covers HHA level home care patients on a daily basis for permanent short term cases and to cover vacations and staff call-offs, assists in the office as deemed necessary by the Director of Patient Services. Guaranteed 30 plus hours per week, flexible schedule, on call 2 weekends per month. Must have reliable transportation and good communication skills. Current NYS CHHA certification.

RN Supervisor
Full-time position 3  – 11 p.m., may be required to do some occasional night shifts. Generous weekend and off-shift differential. Accountable for supervision of patient care within LTC / BI / Sub Acute units. Requires NYS RN license and 2 years experience. BSN, CPR / AED certified preferred.

Admissions Intake Screener
Full-time RN wanted to screen and facilitate patient transfer from the acute care setting to sub-acute programs including short-term rehab, brain injury, long term care. Experience in the hospital setting and/or skilled nursing facility required. Case management, discharge planning and knowledge of payer guidelines preferred. Current NYS RN License required.
